đź©ş Position Summary:

As a Temporary Home Care Registered Nurse (RN), you will provide skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and quality-of-life improvement. This role involves patient assessments, care planning, medication administration, and close communication with families and healthcare teams. Temporary RNs support coverage needs due to census surges, leaves of absence, or weekend/holiday demand.

âś… Key Responsibilities:

  • Perform comprehensive head-to-toe assessments and develop/update individualized care plans

  • Administer medications, treatments, and wound care as prescribed

  • Monitor patient progress and document clinical observations in EMR

  • Educate patients and caregivers on disease management, safety, and medication adherence

  • Coordinate with interdisciplinary team (PT, OT, HHA, physicians) to ensure continuity of care

  • Provide end-of-life support or hospice-level care as appropriate

  • Comply with agency, state, and federal documentation and care standards

  • Participate in RN supervisory visits for aides (if applicable)

  • Report urgent changes in patient condition to appropriate parties

🎓 Qualifications:

  • Active New Jersey RN license (unencumbered) — required

  • Minimum 1 year of RN experience, with at least 6 months in a home health or community setting preferred

  • CPR/BLS Certification — required

  • Experience with EMRs (e.g., HomeCare HomeBase, WellSky, or MatrixCare) — preferred

  • Valid driver’s license and reliable transportation

  • Comfortable working independently in a home environment

🛠️ Preferred Experience:

  • Skilled nursing (wound care, catheter care, IV administration)

  • Medicare-certified home health knowledge

  • Familiarity with Oasis documentation — a plus but not always required

  • Geriatric, palliative, or post-acute care experience

🗓️ Typical Schedule:

  • Flexible assignment durations: 2 to 12 weeks

  • Weekday shifts, per-visit assignments, and weekend availability as needed

  • Minimum 2-hour shift required for hourly assignments
